Report an Incident
You can file a Complaint or Report to the OSIC by submitting a secure, interactive online intake form.
Please note that the OSIC only has the authority to address matters related to violations of the Universal Code of Conduct to Prevent and Address Maltreatment in Sports (UCCMS) that are alleged against covered participants of Program Signatory organizations.
If your Complaint or Report is about conduct that occurred before the UCCMS was in place, it may or may not fall within the OSIC's authority.
If you are unsure of whether your Complaint or Report can be addressed through this process, you may consult with the Abuse-Free Sport Helpline.
You can submit a Complaint/Report form even if you are not sure of whether the matter falls under the OSIC's authority. After your Complaint/Report form is received, the OSIC will review it to determine whether it has the authority to address the matter you've described.
The main factor it will consider is whether the complaint is made against a person who is subject to the UCCMS and if that person is part of an organization that is subject to the OSIC’s process. For more information about the OSIC's jurisdiction and authority, click here.
If you are unable to access the Complaint/Report form, please contact us. The Abuse-Free Sport Helpline is also available to assist you in completing the form.
